Criteria

1. Covered Diagnosis:

Life-threatening Illnesses; including but not limited to (cancer, heart disease, stroke, chronic lung disease, liver disease, AIDS, neurological conditions, diabetes etc.)

2. What We Fund:

Medical expenses; including but not limited to (mammograms, x-rays, screening, lab work, etc.)
Transportation (Travel expenses for treatment - including fuel/mileage, airfare, etc.)
Caregiver expenses
Living expenses
Nutrition

3. Applicant Eligibility Requirements:

Must be a member of Grove Baptist Church 5910 West Norfolk Road, Portsmouth, Virginia or a resident within Hampton roads Virginia.
Must provide individual or family's adjusted gross income
The Primary Care Doctor and the patient will have to sign and attest to the diagnosis
When an application is submitted, you must be about to begin or undergoing a treatment plan in accordance with the prescribed plan of care
Applicants must be 18 years old or the legal guardian can submit the application

4. Award details:

Maximum award amount: $1000.00 per year
Applicants can apply:
- Once every 6 months
- Not more than 2 times in a twelve month cycle
Application processing will be completed within 10 business days. If additional information is needed, the applicant will be contacted within 10 business days.
Checks will be made directly to the verified vendor
Referrals may be made to other sources
All applications will be reviewed for approval or denial of funding by an assigned committee
